What to Expect?
Before your first visit to us at Zenith Integrative Medical Centre, you will complete some extensive medical history questionnaires. The information you provide lays the groundwork for our investigation into the root cause(s) of your current chronic condition. This forms are to be sent to you after an initial deposit to indicate your commitment.
The extensive medical history questionnaire will ask you about your:
Lifestyle History : Including your diet, activity level including sex, work life, hobbies and stressors.
Genetics/Family History: Including your family tree history of physical and mental conditions.
Environmental Exposure History: Including your exposure to toxins and allergens.
Past Medical History: This will include your dental history of implants, heart surgeries, cancer treatments, mental health treatment and any other relevant medical history
Current Medical History: This history will include your current medical treatments both of conventional and other forms of complimentary treatments you are currently receiving.
You will send these completed forms to the practice and you will be advised through a correspondence to call the practice to make your fist booking.
During your first appointment, you’ll have a one-on-one visit with your functional medicine practitioner. Your first visit may lasts from between one to two hours depending on the complexities of the health issues. Sometimes this session will have to be spaced to allow for the work flow of the practice. All this exploration is needed to gives us time to:
- Review your health history and all your current medications (prescribed and over the counter).
- Perform a thorough physical exam.
- Undergo Vital Scan Screening. There is a different brochure that explains to you all you need to know about Vital Scan(ANS testing) & SudoCheck.
- Order lab and imaging test as required.
- Order specialized blood tests as needed.
- Introduce you to our “food as medicine” philosophy.
- Identify your health goals.
- Discuss your results, treatments and follow-up care.
- Develop a personalized management plan document that gives you a framework of action plans. You plan will be reviewed periodically as you journey toward wellness and vitality.
